Session Evaluations
These are used to ensure you receive credit for Continuing Education (CE). For each session you attended, you must fill out an evaluation as well as have signed in on the room sheets and have the room monitor sign your Overall CE form. If you took your CE form home, you can mail it to the office (NASAP, 117 Hedgewood Lane, Canton, GA 30115) or take a picture of the front and back and email them to executivedirector@alfredadler.org. Make sure your name is printed legibly on the front of the form before you mail it or take pictures and email them. Please be sure the pictures are clear and easy to read the session title, presenter, and verification signature. The sooner you complete the evaluations and send in your Overall CE form, the faster you will receive your certificate.
